Supplemental First Payroll Forms are to be submitted at the time of the first certified payroll submittal.

Please ensure that the following forms are attached before clicking the Submit button below. Blank copies and templates of these forms can be downloaded. If a form is omitted (for example, if it is not applicable to the current project), please note such in the comment box below. Attachments must be in PDF format with relevant file names.

  • Letter of Authorization - For use by anyone other than the owners or members of the company signing payrolls for this project, and signed by one of the owners or members.
  • LS-15 - Authorization for Deduction, if applicable.
  • CSV LS-17 - Point of Contact Form (include all relevant fringe benefit agencies with corresponding contact/plan information)
  • Most recent remittance statement for fringe benefit agencies
  • Project Wage Classification and Additional Classification - Form B
