Community Development Block Grant - Davis Bacon Wage Forms Submittal Page

Construction-based general contractors working on Davis Bacon Wage eligible worksites are required to comply with federal submittal requirements for their work. The required submittal forms and deadlines for Davis Bacon Wage eligible projects are listed below.

Blank versions of the required submittal forms, sample documents, and other reference information are available for downloaded from the following zip file: CDBG Davis-Bacon Required Contractor Forms.

Note: This page is not the location to submit contract-bid related documents as part of the City's official procurement process. Please contact the project administrator for information about relevant bidding procedures and submittal forms.

Submit Forms

Pre Award Forms - To be submitted by the bidder with bid documents or within three (3) days of contract award.
Wage Rate Classification Request and Contract Schedule - To be submitted post-contract award (as required).
New Hire Form - To be submitted if prime or subcontractor hires a new employee.
Certified Payroll Submittal - To be submitted within seven (7) days of the date the contractor pays the employees on the subject project.
Supplemental First Payroll - To be submitted at time of first certified payroll submittal.
Supplemental Final Payroll - To be submitted at the time of final certified payroll submittal.
